1. Doesn't really matter - anything I can store for later review, or take with me. Streaming is the only option that doesn't allow that.
2. I have never purchased one in 30 years of practice

4. I own all the software to rip DVDs into other formats, so currently a DVD is fine, but this might change next week so I would suggest planning flexibility in your endeavors!!!
