Terms & Conditions of Engagement Forms – DUE April 13
Faculty participation forms were emailed to course faculty at the end of March. Each page of the
Terms and Conditions of Engagement must be signed in addition to the Hand Society's
Conflict of Interest form for reporting any commercial or financial relationships. All faculty forms are due April 13, 2009. Return your completed form by fax to 847-384-1435. To request a copy of your TCE and/or COI forms to be sent again please email
droesmann@assh.org (please reference the precourse number in your communication).
Presentation Handout – DUE June 22
One of your responsibilities as a faculty member is to submit a handout for inclusion in the course syllabus. It is strongly recommended that your handout follow your presentation and provide detailed information. Completed handouts should be submitted electronically to droesmann@assh.org by June 22, 2009. If at any time you believe you will not be able to meet this deadline please contact the ASSH as soon as possible to work out an alternate schedule.
