See below for more information on each of these important dates. All forms should be emailed to Diana Shkap, Meetings Coordinator, at firstname.lastname@example.org or faxed to 847.384.1435.
Required Forms - November 7
All faculty are required to complete a Conflict of Interest
disclosure form and a Terms and Conditions of Engagement (TCE)
form. Please complete these forms as soon as possible if you have not already. Contact Diana with any questions. Handouts - December 12
All speakers must prepare and submit an electronic presentation and syllabus materials. Lab moderators and demonstrators are not required to submit a handout. Read the handout policy
for guidelines, tips and the handout template.
Pre/Post Test Questions - December 12
All speakers must prepare 1-2 multiple choice test questions relating to their assigned topic. Please include five answers and be sure to indicate the correct answer.
Hotel Accomodations - December 12
Faculty hotel accommodations will be made at the Hyatt Regency New Orleans. Please submit your hotel request form with your flight information and duration of stay and a confirmation email will be sent to you.
Faculty Dinner RSVP - December 12
All faculty are invited to attend the faculty dinner, to be held Friday, February 24. Time and location are TBD.
Speaker Reimbursement - March 12
ASSH will reimburse faculty for any travel expenses for the duration of the course, including airfare, transportation, lodging and meals. Submit your reimbursement form, with original copies of receipts, by the deadline to receive full reimbursement.
