Every so often, a patient presents with a tender firm nodular lesion on the volar aspect of the proximal phalanx that may appear like a small tendon sheath ganglion.  What is different, however, is that it may have a bluish hue or be elongated in shape. I have removed several only to find a thrombosed vein.  Subsequently, I have treated them with beneign neglect and they have resolved spontaneously.  The proper diagnosis is digital varix.
